PACESETTER: Program to Avoid Cerebrovascular Events through Systematic Electronic Tracking and Tailoring of an Eminent Risk-factor

The Program to Avoid Cerebrovascular Events through Systematic Electronic Tracking and Tailoring of an Eminent Risk-factor (PACESETTER) is an implementation trial (at the patient level) of a multi-level integrated approach to stroke prevention through personalized text messaging and home blood pressure monitoring versus usual care in 200 recent stroke patients with hypertension (a majority of whom will be African American), recruited across three main safety net hospitals in Charleston, Columbia, and Greenville, South Carolina. The primary outcome will be achievement of guideline-recommended systolic blood pressure control at 12 months. The intervention, if proven effective and implementable, has the potential to be exported to other medically underserved populations in the United States.
